Support Co-ordination

Emerald Home & Community Care’s experienced support coordinators provide you with best assistance to interpret and connect to the services of your choice, matching with your NDIS plan. Participants are assisted to exercise their rights to choice and control over the service providers. The support coordinator does referrals to the service providers, facilitates service agreements and liaises with support workers, allied health professionals, family, friends and nominees for the best customer satisfaction and quality care to be realised and achieve the goals.
